Consent to the personal data processing

I agree with personal data processing to the following extent: academic degree, name, surname, address, cookies, telephone number and e-mail address in compliance with Act no. 18/2018 Coll. on Personal Data Protection and any subsequent amendments and supplementations (hereinafter referred to only as the “Act“), by the commercial company iClinic plus s.r.o., with its registered office at Dlhé Diely I/6A, 841 04 Bratislava, registered with the Company Register of the District Court of Bratislava I, Section: Sro, File no.: 53996/B (hereinafter referred to only as the “Controller“), for the purpose of recording demands and planning examinations and for the purpose of direct marketing – sending special offers and news.
 

I am granting the consent for an unlimited period of time or until the consent is withdrawn in writing in the same manner as the consent was granted or in another suitable manner delivered to the Controller´s address. I am aware of the fact that subsequent withdrawal of the consent does not affect the lawfulness of personal data processing based on this consent prior to its withdrawal.  
 

The consent to personal data processing has been granted voluntarily, with the awareness of all rights under the Act pertaining to the data subject as well as of the right to lodge a complaint with the supervisory authority.
